Older american act (oaa)- title 3b, c1, c2, 3e - fosters the development of community based systems of service for older persons and provides supportive services (meals, homemaking, personal care, access) along with services to caregivers. This also includes meeting nutritional needs of the seniors under title c1 and c2 by providing nutritional services in a congregate meal site or by meals delivered to the seniors home. 3,380 clients were served.
Aging and disability resource center/medicaid waiver programs-the adrc provides coordinated, multi-access "one-stop" system that integrates information, referral and eligibility determination functions for elders, persons with developmental disabilities, and caregivers while working with the medicaid waiver programs. The waiver program provides alternative, less restrictive long-term care options for elders who qualify for skilled nursing-home care. These options include in home, or in community settings such as assisted living facility or adult day care center rather than in an institutional setting such as skilled nursing facilities. Other programs include nutrition services incentive program, shine program, medicare beneficiary outreach and assistance, chronic disease self management and oaa d1 health, wellness and medication management. Home energy assistance - the emergency home energy assistance program (eheap) assists low income households with at least one member 60 years of age or older experiencing a heating or cooling emergency. Home energy assistance program helps seniors to pay for their energy bills up to 5,000 per year and also can assist with replacement or repair of air conditioning units. Shine is serving health insurance needs of elders, and assists them with their medicare and medicare advantage plan. Information and intake refers to the elder helpline services aaa provides, connecting seniors to care, which is a part of the oaa program (4a). Served 9349 clients.
General revenue - cce, hce, adi, respite relief - provides in home services to the low income, frail seniors over age 60 with the goal being to reduce or delay premature or inappropriate and expensive placement of older persons in nursing homes and other institutions. Seniors were able to remain in their own homes longer by participating in these programs. By helping a senior remain at home their quality of life is better and the cost of support care is less costly. Adi served 351 seniors, cce served 2,993 seniors and hce served 210 seniors.
Vdhcbs - veterans home based direct care program allows for veterans who need daily living assistance to be assigned a caregiver and live in their own home, served over 60 clients.
