Clinical/Medical Services Apicha Community Health Center CHC is a Patient-Centered Medical Home as recognized by the National Committee on Quality Assurance. Since 2020, when the COVID-19 pandemic struck, we have adopted a hybrid service delivery model, offering both telehealth and in-person services to meet patient care needs while ensuring staff safety. As a Federally Qualified Health Center FQHC, Apicha CHC provides a full range of medical, preventive health, and supportive services to all, regardless of ability to pay. Medical services include primary care for adults and children HIV medical care PrEP/PEP Transgender Health Womens Health Dental Care Mental Health and an onsite pharmacy at our Manhattan clinic. Support Services include HIV Support Care Coordination and Nutrition Education Transgender Health Support PrEP/PEP Coordination Services Medical Case Management and Health Home Medical Care Management Services for people with chronic diseases.
