THE ALZHEIMER'S EARLY DETECTION PROJECT is a memory screening assessment, prevention, and treatment program for seniors with risk factors of cognitive memory decline. The program provides free cognitive memory screening exams, disease education, caregiver education and referrals of at-risk patients to physicians for follow-up assessment.
Alzheimer's Care Project is an in-home assessment, treatment & management program for people living with Alzheimer's disease or related dementia that are home bound. The program is intended to provide assistance & comprehensive care to seniors living at home with dementia & those who care for them over the course of their illness; to age in place with a maximum of independence.
ALZHEIMER'S AND DEMENTIA SUPPORT SERVICES - a caregiver support group and crisis management support program to create and prevent burnout, reduce levels of strain and depression, and increase caregiver capacity with the primary intent on delivering individual Crisis Management Support. Support includes immediate help on ADRD behavioral interventions, neglect and abuse, wandering, theft, rehousing, depression, burnout, referrals, and other complications that arise and threaten the treatment of ADRD.
ALZHEIMER'S PREVENTION EDUCATION PROJECT The Alzheimer's Prevention Education Project is a group awareness and educational program that addresses 3 categories of individuals closest to the Alzheimer's epidemic including seniors, caregivers, and individuals who have been diagnosed with dementia.
FOOD4LIFE PROGRAM Targeted solution to ensure that food-insecure and socially isolated older adults with barriers to food access can stay fed and healthy during the Covid-19 Pandemic.
