Program areas at Unison Health
Substance use disorder services - our sud services provide a full continuum of care for addiction, including outpatient treatment, mediciation management, residential treatment and a sober living facility.
Medical services - our medical service team provides psychiatric, medical , nursing and education services as well as medication management to our patients with psychiatric conditions.
Intensive mental Health services - we provide treatement for the spmi population (severe and persistently mentally ill) clients. We provide coordination of care for these clients which integrates the physical Health, mental Health, long term care and social service needs. We also provide assertive community treatment (act) which is a multidisciplinary team approach with assertive outreach in the community. This person-centered care model reduces hospitalizations, increases housing stability and improves quality of life for people living with mental illness.
School based care - uison is contracted with local school systems to provide programming with the school to improve classroom performance and encourage emotional self-control. Unison also provides 24/7 crisis care for psychiatric emergencies in wood co. and a samsha grant funds the provision of mental Health services within the lucas co jail, at the cherry st missions (homeless shelter) and further expands services in the schools.
Other program services include residential and mobile crisis support.
