Reviews and counsels participants on screening results 4. Provides participants with identified community medical resources for intervention treatment 5. Provides outcome screening data to hospitals that include high risk participant screening results and contact information for hospitals to follow-upbeyond the hospital program: 1. Educates inpatients on secondary Stroke prevention and on post -Stroke issues 2. Provides education manual for interdisciplinary patient education, according to individual treatment assessments 3. Provides three-month follow-up interview using evidenced based criteria in Stroke prevention to measure education effectiveness and equality outcomes 4. Provides objective outcomes that includes patient medication compliance, on re-hospitalization, and on patient level of Stroke disability 5. Provides construct for any hospital to implement quality improvement and improve Stroke care in North Carolina 6. Provides each hospital with profile and outcomes reports
