Mayo Clinic Health system is a family of clinics, hospitals and Health care facilities serving communities in Minnesota, Iowa, Wisconsin and Georgia. As part of Mayo Clinic, the Health system offers care designed for the whole body close to the patient's home, when they need it. A multidisciplinary team of experts has ready access to knowledge, resources, and expertise of Mayo Clinic and collaborates on providing exactly the care the patient needs. Mayo Clinic Health system sites employ approximately 1,000 physicians and 15,000 allied Health staff. In addition, Mayo Clinic physicians from multiple specialties regularly see patients at Health system facilities. Besides having Health care facilities in nearly 70 communities, Mayo Clinic Health system includes 19 owned hospitals, five owned nursing homes, physician service agreements with one additional hospital, and contract management agreements with two additional hospitals and one nursing home. Since its inception in 1992, Mayo Clinic Health system has grown into one of the most successful regional Health care systems in the united states. More than 600,000 unique patients account for more than 2.6 million visits annually in Mayo Clinic Health system. It is also a national leader in quality and safety efforts. Mchs-chippewa Valley is a Wisconsin nonprofit corporation, tax exempt as an organization described in section 501(c)(3) of the internal revenue code. The corporation, which includes a hospital, clinics and a nursing home, operates primarily for the residents of bloomer, Wisconsin and surrounding communities.mchs-chippewa Valley's mission statement states that the organization's aim is to inspire hope and contribute to Health and well-being by providing the best care to every patient through integrated clinical practice, education and research. Our vision is to achieve the highest standards for medical care and Health improvement in the communities in which we live and work. Our first priority is to meet patient needs guided by the core values of integrity, continuous improvement, service, teamwork, innovation, balance and financial integrity. As a member of Mayo Clinic Health system, mchs-chippewa Valley provides quality healthcare close to home with direct access to highly specialized care if the need arises. Mchs-chippewa Valley operates on a nondiscriminatory basis, regardless of race, color, sex, religion or national origin.the hospital plays a vital role in the Health and well being of the community by offering services such as 24-hour emergency care, same day surgery, pt/ot/rt/speech therapy, pulmonary rehabilitative services, cardiac rehabilitative treatments and advanced diagnostic technology.the hospital had 429 admissions in 2014 including adult inpatient and swing bed admissions. There were 1,911 emergency room visits in 2014, along with 119,078 laboratory procedures, 13,151 therapy and rehabilitative services, 450 outpatient surgeries and 11,790 imaging procedures performed. Mchs-chippewa Valley is licensed for 25 acute and 35 long-term care beds. In 2014, mchs-chippewa Valley discharged more than 420 patients. Each resident of the nursing home receives personalized medical, social and spiritual care. The focus is on preserving the lifestyle of each resident with 24-hour nursing care, social services, activity therapy, dietary services, chaplain services and rehabilitation therapy. There were 10,366 days of service in the nursing home during 2014.financial assistance is available to patients financially unable to pay for services provided. The cost of financial assistance provided in 2014 was approximately $453,000.mchs-chippewa Valley also provides care to persons covered by governmental programs. Services are provided to both medicare and medicaid patients at substantial discounts from standard fees.mchs-chippewa Valley provides support services to affiliated organizations to help further their exempt purpose.the northwest Wisconsin region of the Mayo Clinic Health system (which includes the filing organization) voluntarily entered into an agreement with the Wisconsin attorney general relating to charity care. The terms of the agreement include an automatic discount for uninsured patients, a payment cap for those uninsured patients that meet the organization's charity care criteria, and a commitment to make information available to our patients regarding our charity care program. Additionally, the Wisconsin attorney general reviewed and approved our billing and collection policies and practices.
Community benefit (schedule o):mchs-chippewa Valley's community efforts include classes and various department presentations open to the community at no cost. A number of different disciplines were involved in the provision of these offerings. Additionally, a community wellness committee was established and tasked with developing more wellness/education offerings for the community to begin in late 2013, early 2014.in 2014, mchs-chippewa Valley provided education to students (kindergarten through high school) about hand washing, healthy eating and exercise. Mchs-chippewa Valley donated to various school functions such as after-prom parties, run/walks, mentor programs, sporting fundraisers, as well as safety net and fired up programs. The facility also helped with the local school supply drive.during 2014, mchs-chippewa Valley facilitated two support groups: domestic/sexual abuse (1 contact) and breast cancer (80 contacts).in addition to support groups, mchs-chippewa Valley offered a number of heartsaver cpr and aed and friends and family cpr classes at a free or low cost to the community.in 2014, a pilot program was established to provide blood pressure screenings and Health information at the bloomer food pantry. This pilot has reached over 120 contacts in 2014.mchs-chippewa Valley continues to work towards youth education by providing four growing up classes a year and numerous babysitting training classes. These classes cover Health and wellness topics for adolescents and have reached over 140 children.the facility continues to build partnerships in the community to offer evidence based classes such as stepping on, a fall prevention program.mchs-chippewa Valley continues to help fund and staff the bloomer jump rope contest, an annual community event held in january. Mchs-chippewa Valley set up a booth and are sponsors of the colfax fair, the bloomer fair and the northern Wisconsin state fair. Staff also participated in parades in bloomer, chippewa falls and colfax during the summer months. During these events, staff promoted Health and wellness through teaching proper hand washing and encouraging movement.our facility is a corporate sponsor for the chippewa county relay for life walk. Staff coordinate and fundraise for this event each year raising approximately $3,000 in addition to the corporate sponsorship and participating in the relay for life walk. Staff also participated in the chippewa Valley heart walk and the making strides against breast cancer walks, both held in eau claire and both raising over $2,000 for those causes.a safety camp was held in chippewa falls for 4th and 5th graders to promote Health and safety to the 44 children in attendance. In october, a Health fair was held in bloomer in conjunction with the community women's club arts and crafts fair for the eighth year with attendance being around 350 people. A fall women's Health fair was also held at the Clinic in colfax and about 30 people attended. Various booths, information and screenings are conducted throughout the year within the surrounding communities as well.in 2014, the facility was also an active participant in: chamber of commerce events in bloomer and chippewa falls, wellness events at the aquatic center in bloomer and donating to area food pantries. Four families were also sponsored and provided with christmas gifts during the holidays.mchs-chippewa Valley provides an instymeds machine in the facility to provide patients access to emergent medication prescriptions generated from the emergency department when retail pharmacies are closed. There were 978 prescriptions filled thru instymeds in 2014.mchs-chippewa Valley has a volunteer program comprised of approximately 50 community members. Community volunteers assist activity, Health information, social services, hospital nursing and administration departments. The volunteers assist by helping plant flowers, greet patients and assist residents during activity programming. The volunteers are very instrumental in helping with fundraising events for the medical center. About 4,357 hours were donated in 2014 at an approximate value of $100,516 based on statistics from the independent sector.in addition, mchs-chippewa Valley donated to local charities and community organizations during 2014.
Health professional education (schedule o)under the Health care administration practicum program, mchschippewa Valley provides internships for students from the university of wisconsin-eau claire. The interns, under the direction of nursing home administration, works with employees to gain knowledge and experience in healthcare administration. In 2014, two students had the opportunity to learn about the Health care environment.also in 2014, mchs-chippewa Valley awarded $2,500 post secondary scholarships to local high school students pursuing careers in a health-related field.
