Program areas at Human Resources Consultants
Transitional community opportunities for recovery and engagement. This is a transitional program to bridge the gap in service delivery from acute care services or jail psychiatric services to regional support teams or other outpatient services. In this transitional period, clients need to be provided with mental health services to maintain stabilization and prevent further crises. This program is highly successful in getting clients engaged in services in order to assist them with their mental health challenges.
The quality and comprehensive services offered by both programs contribute to the stabilization and increased quality of life for many of our clients. A number of our clients have graduated from our programs and have found success in employment, going back to school, and reuniting with family members.
Coordinated care services and provided comprehensive services to those persons identified as acutely and persistently mentally ill. services include: assessment; intake and referral; crisis intervention; medication services; development of an individual treatment plan. Referrals to appropriate agencies for employment, income and housing assistance; life skills training; substitute payee services; referrals to supportive housing; consultation, education; information and support services to families; coordination and linkage with other agencies; and counseling for the purpose of preventing decompensation and/or admission to higher levels of care.
