Behavioral health, Community health and wellness Services provided to 13,333 clients in Home Nursing Agency's southern central Pennsylvania market area. For the year ended june 30, 2014, Home Nursing Agency Community service provided 839,243 of uncompensated care to its behavioral health, Community health and wellness clients. The behavorial health function did receive 3,442,228 of contributions, gifts, and/or grants in the year ended june 30, 2014, reported in line 1 of part viii, not included in the revenue total above, per part iii instructions. See schedule o for description of methodology to compute uncompensated care. Whether it's at Home, school, or at one of our Community sites, hna Community Services are committed to ensuring families have the resources to remain in their homes and in their communities. Over the past year alone, the hna cs staff has served as leaders, and/or are currently serving multiple boards, committees, and Community grass roots associations including head start health advisory committee; Pennsylvania Community providers association; nurse-family partnership supervisors and administrators groups, as well as the following local county-based efforts: blair county: healthy blair county coalition lifestyle committee and early childhood sub-committee blair county health and welfare council, marketing director penn state cooperative extension advisory board blair county social Services advisory board may is mental health month committee blair county human rights committee, chair blair county providers association, co-chair local intra-agency coordinating council (licc) maternal child symposium planning committee teen pregnancy prevention team week of the young child cambria county: cambria county health and welfare council communities that care committee cribs for kids committee executive board committee local intra-agency coordinating council (licc) child find committee penn cambria school district literacy team cambria county child death review team laural highlands breastfeeding coalition cambria county week of the young child centre county: centre county council for human Services local children's team cenclear health advisory committee centre county partnership for Community health and obesity/diabetes, sub-committee chamber of business and industry of center county local interagency coordinatiing council (licc) leadership centre county centre county child death review team clearfield/jefferson counties: local intra-agency coordinating council (licc) kidd council huntingdon county: huntingdon county healthy communities partnership huntingdon county human service council huntingdon county prevention network early head start planning/ continuous improvement committee 2013-14 accomplishments of hna cs behavioral health, Community health and wellness. Partnering with the local national alliance on mental illness (nami) chapter and county mh/mr, hna cs continues to offer ancillary service called 'opportunity drop in' which welcomes any individual in the Community that lives with a diagnosed mental health disorder (including co-occurring) to socialize with their peers. Held weekly, over 60 individuals are now 'dropping in' at this highly valued peer-run service offered to the community-at-large. Hna cs school-based children/adolescent program serves over 700 students from 11 school districts, throughout 59 satellites. 100% compliance from pa department of health/welfare: - tartaglio personal care Home - blended case management - adult case management - peer support Services - center for counseling Home Nursing Agency programs/services including behavioral health, drug & alcohol, early intervention, and nurse-family partnership, featured in regional newspapers and publications expanded early intervention program to centre county all behavior specialists in the children's Community health center have become licensed to provide Services to those diagnosed with autism began training under pcit (parent child interaction therapy) across Pennsylvania study being conducted by the university of pittsburgh adult center for exceptional learning (acel) hosted an artist-in- residence program in the summer that will result in a mini exhibit at the southern alleghenies museum of art in altoona maintained at least 95% capacity of clients in the nurse-family partnership in blair, cambria, and huntingdon counties statement of usage of support funding from Home Nursing Agency foundation: aids intervention project - funds used for personal care/hygiene items, cleaning supplies, vitamins and dietary supplements and assist homeless individuals and families dealing with hiv/aids. Nurse family partnership - funding to provide support to this evidence-based Community health program that transforms the lives of first-time mothers living in poverty by improving pre-natal care and parenting skills resulting in child's healthy growth and development and to improve self-sufficiency. Nurse family partnership serves seven counties, providing care to 300 - 600 families each year. Acel - adult center for exceptional learning - dollars to support the continuation of the southern alleghenies museum of art (sama) artists-in-residence program that provides acel individuals who have cognitive and physical disabilities the opportunity to experience art, painting, dancing, and storytelling, which can improve their quality of life. Acel - adult center for exceptional learning - funds for individuals who attend the day program but who go over his/her funding allocation cap as well as help those who have no funding available and cannot self-pay for Services. Blended case management/resource coordination - for clients of the behavioral health program who have unexpected emergencies or when an individual has minimal income and needs beyond normal circumstances. Also helps fund household and personal hygiene products, clothes for employment/school and bus passes for those starting employment. Blended case management/children and adolescents - to be used for children and adolescents who are enrolled in the children's behavioral health program who are in need of personal/hygiene products, cleaning products, food, seasonal clothing items and other daily living needs. Also to be used for start up costs for Community and school based activities, such as intramural sports, boy scouts, girl scouts, and used in situations to assist children and families with no or limited income. This program currently serves more than 400 children, adolescents and families. Adult group blair (art therapy) / adult partial hospitalization (art therapy is used in treating post-traumatic stress disorder, anxiety disorders, and depression and self-esteem symptoms. The partial hospital program - offers intense therapy to individuals who are severally depressed, anxious, or experiencing psychosis.) - dollars to support adult behavioral health program clients utilizing the art therapy program as part of their treatment plan. Funding to support art supplies used in weekly art therapy sessions and an end-of-year art show recognizing their art. Center for counseling - funds to assist in providing treatment to more than 300 children and 1,000 individuals who are experiencing behavioral health issues and who otherwise would not receive Services due to having no insurance, high co-pays/deductibles, or private insurances would not cover counseling Services. Multi systemic therapy (mst) - mst is an intensive family and Community based treatment program that focuses on encompassing the environment of the chronic and violent juvenile offender. Funding to purchase gas for families to attend appointments (psychiatric, psychological, school and court hearings), seasonal clothing, basic household items, such as bedding and cleaning supplies and groceries. Drug and alcohol adult partial hospitalization/d & a adult outpatient - funds to help individuals seeking drug and alcohol treatment that would otherwise not receive Services due to having no insurance, high co-pays/deductibles, or private insurances that will not pay for d & a Services. Adult partial hospitalization (offers Services such as group counseling (including art therapy) accompanied by psychiatry to aid the individual in remaining independent in their Community and out of the hospital)- dollars to help individuals with a mental health diagnosis who attend the program on a daily basis and who cannot afford co-payments related to insurance, county liabilities or financial difficulties. Children and adolescents partial hospitalization (blair & centre counties)- funds to stock the "reward store," for children and adolescents enrolled in the school-based behavioral health program that utilizes a therapeutic incentive-based model that changes negative behaviors into positive ones with the use of a rewards system. Therapeutic staff support (tss) (provides therapeutic support to polar disorder and attention deficient hyperactivity disorders)
Private duty Services provided 404 clients in the Pennsylvania counties of blair, cambria, bedford, huntingdon and centre with living assistance and supportive Services. An additional 337 clients are supported by rental of personal response devices. For the year ended june 30, 2014, Home Nursing Agency Community service provided 113,817 of uncompensated care to its private duty clients. See schedule o for description of methodology to compute uncompensated care. Private duty Services help residents to avoid premature placement in a long-term care facility by providing the support they need to remain in the comfort, safety, and privacy of Home while offering direction and peace of mind to family members who have other obligations or live at a distance. Whether they need a few hours of assistance a week or 24/7 care, our personalized non-medical Home care Services allow them to receive the level of support needed while providing the dignity and respect they deserve. 2013 -2014 accomplishment of hna cs private duty: private duty Home care Services earned the 2014 best of Home care award in 10 of 12 categories
Adult day Services provided 57 clients in the Pennsylvania counties of cambria and bedford with supportive day care. For the year ended june 30, 2014, Home Nursing Agency Community service provided 60,789 of uncompensated care to its adult day clients. The adult day care function did receive 23,687 of contributions, gifts, and/or grants in the year ended june 30, 2014, reported in line 1 of part viii, but not included in the revenue total above, per part iii instructions. See schedule o for description of methodology to compute uncompensated care. Home Nursing Agency strives to maintain or improve the quality of life for individuals, 18 years of age or older, who have physical limitations, memory loss, or who have difficulty remaining safe at Home alone during the day. The adult day program also yields a significant benefit to caregivers who will have some time to tend to their own personal needs while their loved one is in the care of the hna cs's adult day staff. In most instances, transportation is also arranged. Improving the health and functioning of every adult day participant is the number one goal at hna cs, and a personalized plan of care is developed for each participant. The medical, social, and nutritional needs of each participant are met every day or as needed. Often, a client's memory and mobility are improved and they enjoy improved rest in the evenings - all significant benefits to the caregiver as well. Caregivers need breaks and adult day centers provide support and relief. Services include the following: safe environment personal care medication management socialization nutritious meals monitoring of ongoing medical conditions. 2013 -14 accomplishments of adult day Services: 100% compliance from pa department of health/welfare: - ebensburg adult day Services statement of usage of support funding from Home Nursing Agency foundation: adult day Services - to provide additional days of support to help families successfully manage the caring of a loved one who is living at Home and is unable to independently perform self-care activities or function alone safely. Adult day Services can prevent or delay institionalization through medical monitoring, social and recreational activities and personal care. Adult day Services - to support the southern alleghenies museum of art (sama) artists-in- residence program that provides adult day Services individuals with art experiences, i.e. Painting, dancing, and storytelling to improve thier quality of life.
