Program areas at Christiana Care Health System
Christiana Care Health Initiatives provides charitable primary Care, home-based primary Care to homebound patients, cardiology, substance abuse and other healthcare-related services to the community in support of the Christiana Care Health system. Primary Care services were provided to the salem county community in new jersey. Providers within two primary Care practice locations performed a total of 13,675 in-person office and virtual visits for fiscal 2022. Home-based primary Care services were provided to homebound patients in the new castle county and middletown communities in Delaware. Providers, including both physicians and nurse practitioners, performed a total of 4,978 home visits for fiscal 2022. Cardiology services were provided to the salem county community in new jersey. Providers, including a physician and medical assistants, performed a total of 2,028 visits for fiscal 2022. Substance abuse and other healthcare-related services were also provided to the community during fiscal 2022 in furtherance of the organization's primary tax-exempt purpose and the Christiana Care Health system.
