Hospice Home Care is a program designed to care for terminally ill patients through the use of the Interdisciplinary Team, which includes Nurses, HHA's, MSW's, Medical Director, Clergy, Therapists, Pharmacists, Trained Volunteers and Counselors. Care is provided in the person's place of residence, where ever that may be. Bereavement support is provided to the family for up to thirteen months after death.
