Children's health centersthe children's health centers of Vna area nurse managed primary healthcare centers for uninsured and underinsured children whose familiesfind it difficult to obtain care and would otherwise not seek healthcare except on an emergency basis. Sites are located in abington andnorristown, montgomery county; care is also provided in arearesidential facilities. Care includes: routine well-child care,diagnosis, and treatment of the sick child, and if appropriate,referral to specialist, immunizations, school and sports physicals,parenting education, home visits when necessary and referral support.an important component of the care is assisting families in obtainingfree or low-cost health insurance and other entitlements to support
In-home Services programthe in-home Services program of Vna provides intermittent long-termcare to elderly and disabled individuals to enable them to remainsafely in their own homes and communities. Services include: personalcare, homemaker support, respite for the informal caregiver andcompanionship care.
Personal navigator programthe personal navigator program of Vna is an access and enrollmentprogram that utilizes technology and the assistance of skilled staff toconnect eligible individuals and familities to programs and resourcesavailable to strengthen self-sufficiency and independence. Resourcesavailable include possible free and low-cost prescription medication,emergency food and shelter assistance and medical, housing, utility andfinancial benefits.
