the direct hospice core support Services fulfill the medical Services and admissions requirements into hospice care. Our medical team of physicians provides their expertise in symptom management and comfort care. Physicians are an integral part of the hospice team. the physicians and members of the admissions team educate residents about hospice care and evaluate their medical eligibility to initiate care. Our community outreach requires continuously educating physicians and residents in martin, st lucie and okeechobee counties about how to access hospice Services. Treasure Coast hospice grief support Services (tchgs) provides grief counseling and support to hospice patients, their families and anyone that has experienced a death in martin, okeechobee and st. lucie counties. the counseling service staff are master-level prepared clinicians who are licensed by the state of Florida to provide counseling. Services are provided to adults and children in our offices, patient family homes, schools and community agencies. We break down our Services to: 1) bereaved individuals/families of patients that have received hospice care. During the current year, 8,499 individuals received bereavement support and education through newsletters, anniversary cards and phone calls. 2) we provided support to 257 adults and 18 children whose loved one died on hospice care through individual, group, or family counseling. Counseling Services were also provided to 297 adults and 179 children with respect to a community individual whose loved one did not die under hospice care. In total, 2,960 counseling sessions were provided on an individual basis. 3) crisis response and education are provided throughout our service area when there has been a tragedy or a crisis in our community. There were 605 individuals (adults and children) that received crisis support and education.
Treasure Coast Palliative care (tcpc) has a program that provides consultative Palliative care Services to patients with serious illness. Services provided include clarification of goals of care, establishing advanced directives and discussing treatment options and their expected outcomes. Palliative care consultation Services are provided by a specialized team comprised of physicians and advanced nurse practitioners. These Services were available at local area hospitals, nursing facilities and assisted living facilities.
