Community Healthcare Partners ACO (CHP) is the clinically integrated provider network that unites independent and employed physicians and other providers along with Community Healthcare System's hospitals. This network brings together more than 1,000 area physicians and allied health professionals to collaborate on ways to improve the quality of care, patient satisfaction and efficiency of healthcare costs. The CHP approach is to coordinate healthcare to suit the individual needs of patients by encouraging providers from across the healthcare system to work with one another to assure the right care is delivered at the right place at the right time. We feel this approach is crucial to attaining high quality health care. The use of quality metrics to enhance patient care and outcomes remains the predominant goal of our clinically integrated network. Using sophisticated analytics software, we are able to employ robust quality data that is used to improve care and evaluate CHP's clinical practice guidelines. These evidence based guidelines represent practice that will drive improved patient outcomes and adherence to FTC requirements for clinically integrated providers. The Care Navigation team of Community Health Partners provides outpatient care management services to all members who may benefit from assistance navigating through the healthcare system, from health promotion to complex medical management. One on one support by skilled nursing professionals help to coordinate care of members by identifying gaps in care, encouraging wellness visits and assisting those identified as high risk. Using sophisticated risk stratification tools our care management team is dedicated to providing a continuum of care that not only improves patient care but demonstrates value to our providers.
